1. Avoiding General Anesthesia: A Safer Approach

One of the most significant innovations in 2025 is the widespread avoidance of general anesthesia for BBL procedures. Instead, surgeons are increasingly using the tumescent fluid technique, a solution developed by dermatologist Dr. Jeffrey Klein.

This method involves infiltrating a carefully prepared fluid containing lidocaine and epinephrine into the fat and gluteal area. The benefits are clear:

  • Greater comfort for patients without needing to be fully “under.”
  • Less bruising and swelling, since the solution constricts blood vessels.
  • Faster recovery times, with most patients walking the next day.
  • Improved safety, as patients can respond during the procedure—helping ensure fat is placed in the correct layer.

When the cannula touches a deeper structure such as the gluteal muscle, the patient reacts immediately—alerting the surgeon that they are in the wrong plane. This simple feedback loop helps prevent injections into the muscle, which are linked to dangerous complications like fat embolism (when fat enters the bloodstream and travels to the lungs).

By contrast, under general anesthesia, neither the patient nor the surgeon receives this crucial signal. Most reported cases of fatal fat embolism have occurred in procedures performed with general anesthesia.

In 2017, studies revealed a high risk of complications when fat was mistakenly injected into the muscle, with a reported mortality rate of 1 in 3,448 cases. By 2018, the Aesthetic Surgery Education and Research Foundation (ASERF) issued strict guidelines requiring fat to be placed only in the subcutaneous space (just under the skin, above the muscle).

The results were dramatic: the mortality rate improved to 1 in 14,921. Similar findings were reported by Brazilian surgeon Dr. Cansanção, who calculated a rate closer to 1 in 20,000, and later confirmed by WAGS at their 2022 Miami conference.

With ultrasound technology, surgeons can now see the cannula in real time and confirm proper placement—eliminating guesswork and dramatically improving safety.

Another factor influencing safety is physician fatigue. Performing multiple high-demand BBL procedures in one day increases risks for patients.
